Return Request

If you are not completely satisfied with your purchase for any reason, you may return new and unused products to us for a full refund or credit. To start a return request, please fill out the below form or call customer service at 386-597-7523 to obtain a Return Merchandise Authorization (RMA) number. After receiving an RMA number, place the item securely in its original packaging and include this form when mailing in your return.

Please note that we cannot accept returns for the following items:

• Damaged products
• Used products
• Open and nonsterile products
